It happens all too often. A client takes the time to take care of his or her family’s future, completing a careful estate plan.  Some time later, though, a mistake is made when the client mistakenly goes into a bank, or insurance brokerage, or stock brokerage and fills out a beneficiary designation form nominating someone as beneficiary that conflicts with the terms of his or her will or trust.
